EFMP-Family Support is available to all DoD ID card holders and their family members—including Active Duty, Reserve, National Guard, retirees, and all service branches.

📦 EFMP is a DoD-wide program, and your enrollment transfers with you. Your gaining installation’s EFMP-FS team will be available to continue support.

To disenroll a family member:

  • Request: Contact your local Military Treatment Facility (MTF) EFMP Coordinator.
  • 📝 Provide Documentation: Have a qualified provider complete updated DD Form 2792 and/or 2792-1, or provide legal/dependent documentation (e.g., divorce, age-out, etc.).
  • 📬 Submit: Return all paperwork to your MTF EFMP Coordinator.
  • 📢 Wait: The EFMP enrollment office will notify you once the review is complete.
